TERMS AND CONDITIONS
1. The certificate is applicable for ticketing on US Airways, US
Airways Express and US Airways Shuttle only
2. The certificate my not be transferred of refunded
3. The certificate is void where prohibited by law
4. The certificate is void if altered, copied, damaged or revised in any
way
5. The certificate my not be resold
6. The certificate has no cash value
7. The certificate may not be combined with any other discount program
or coupon offer
8. The certificate is valid for one year from the date of issue
9. Tickets issued in exchange for the certificate are valid for one year
from the date of issue
10. The certificate is void after the expiration date listed on the
front of the certificate
11. Lost, stolen or damaged certificates will not be replaced.
Certificates are non-refundable
12. The purchaser is responsible for the proper distribution of the Take
Flight Incentive Travel Certificate
13. Travel must be via the most direct of applicable route as published
in the current US Airways schedule. En route stop overs are not permitted. Travel must originate and terminate in the
same city
14. A Saturday night stay is required for travel in conjunction with the
certificate
15. Certificates may not be used for business or group travel
16. A description of the proposed incentive program must accompany the
application form
17. US Airways Dividend Miles members may earn mileage credit
18. International departure taxes, federal inspections fees (including
Customs and Immigrations fees), passenger facility charges, infant transatlantic/transpacific fares,
Canadian/Caribbean/European taxes and other surcharges/fees/taxes are the responsibility of the passenger and are
not included as part of the certificate
19. The Passenger cannot pay a fare difference to upgrade to First Class
since certificate has no cash value
20. Some Dividend Miles upgrade certificates may be used. Terms and
conditions of upgrade certificates will apply
21. Seats are subject to availability at the time reservations are made
22. Schedules are subject to change without notice
23. Once exchanged for a ticket, the certificate is deemed fully used
24. Once the ticket is used, the origin and destination may not be
changed. The flight (s) and date (s) may be changed, provided the original restrictions are met. The standard
service charge applies for all changes and ticket reissues
25. Standby will only be permitted on the return portion of the trip

PASSENGER INSTRUCTIONS:
1. Contact US Airways Group and Meeting Travel, toll free, at
1-877-US GROUPS, between 8:00 a.m. and 9:30 p.m. Eastern Time for reservations and ticketing information
2. Advise the sales representative that a US Airways Take Flight
Certificate will be used, and provide the Gold File Number on the front of the certificate. Make the applicable roundtrip
reservation
3. After the reservation is made, present the certificate along with
your flight information to any US Airways ticket counter or City Ticket Office for ticketing. Certificates must be
redeemed on or before the expiration date on the certificate. Unticketed reservations will be cancelled automatically
without notice
4. Certificates cannot be exchanged for prepaid tickets, Tickets By
Mail, electronic tickets or group tickets. Travel agencies, corporate travel departments and other airlines are not
permitted to book reservations or issue airline tickets in exchange for the certificate
5. Certificates must be signed in the presence of the ticketing agent.
The signature acknowledges the acceptance of and compliance with all terms governing the usage of the certificates
